What do you do with out of focus images?

I'm not talking about pictures that have motion blur (that can be desirable to show motion and some people deliberately introduce some blurring),  or out of focus areas outside of your main focus (e.g. Lensbaby), or even camera shake (which can sometimes be corrected in Photoshop). What if your main subject is out of focus? Do you ignore it if the focus is off only by a little (so that you would have to zoom/enlarge to see the misfocus)? What if the misfocus is quite obvious, even when zoomed out?
