Nikon - Lens front and rear focus

Reading lens reviews I sometimes see that the reviewer complains about front or rear focus. Could somebody explain how this can happen with a camera such as a D500. I had understood that the camera adjusts the focus based on an image inside the body - so surely the camera will move the lens element until the focus is correct. Have I misunderstood something.
